He quickly approaches the scene. 5. Immediate Response: • The foreman assesses the situation and confirms that the electrician is injured and in need of immediate assistance. • He activates the emergency response protocol by informing the safety team via radio, providing details of the incident and the location. • The safety team mobilizes quickly, gathering first aid supplies and preparing for the injured worker’s transport. 6. Though inactive since 2020, 176 personnel were onboard for decommissioning preparations. The blaze injured 14 workers and disrupted operations. One individual fell into the sea during the chaos but was safely rescued. This incident adds to a troubling pattern in offshore operations: • Deepwater Horizon (2010): Blowout, 11 fatalities, massive environmental disaster. • Cidade de São Mateus FPSO (2015): Gas leak, explosion, 9 fatalities—most were emergency responders. • P-36 (2001): Explosions, 11 fatalities, platform sank days later. • Trinity Spirit FPSO (2022): Explosion and fire off Nigeria’s coast, resulting in at least 7 deaths and a significant oil spill. Different contexts, but common failures—especially in emergency preparedness and response under pressure. What keeps going wrong? • Emergency teams overwhelmed or underprepared. • Communication breakdowns during critical moments. • Plans that didn’t match real-life complexity. • Delayed actions due to unclear leadership or roles. • Systems designed for control—not for chaos. These are not just technical issues. They are human and organizational challenges that require a different kind of preparedness. If we want to protect lives, we must evolve. Emergency preparedness must be treated not as a checklist, but as a core capability—something that is rehearsed, challenged, and deeply embedded in how we operate. But the real issue was not how it started. The real issue was what happened next. There was 𝗵𝗲𝘀𝗶𝘁𝗮𝘁𝗶𝗼𝗻. There was 𝗱𝗲𝗹𝗮𝘆. There was 𝗻𝗼 𝗶𝗺𝗺𝗲𝗱𝗶𝗮𝘁𝗲, 𝗰𝗼𝗼𝗿𝗱𝗶𝗻𝗮𝘁𝗲𝗱 𝗳𝗶𝗿𝘀𝘁 𝗮𝘁𝘁𝗮𝗰𝗸. Fortunately, the fire did not spread. Fortunately, fuel flow was limited. Fortunately, no one was injured. But “𝗳𝗼𝗿𝘁𝘂𝗻𝗮𝘁𝗲𝗹𝘆” 𝗶𝘀 𝗻𝗼𝘁 𝗮 𝘀𝗮𝗳𝗲𝘁𝘆 𝘀𝘆𝘀𝘁𝗲𝗺. The difference between control and catastrophe can be less than 100 seconds. That first minute is not about equipment only. It is about competence. • Was the emergency stop activated instantly? • Was fuel isolated without confusion? • Was the extinguiser deployed at the base of the fire with confidence? • Was the area cleared with authority? Fire extinguishers mounted on walls are compliance. 𝗧𝗿𝗮𝗶𝗻𝗲𝗱 𝗽𝗲𝗼𝗽𝗹𝗲 𝗮𝗿𝗲 𝗽𝗿𝗼𝘁𝗲𝗰𝘁𝗶𝗼𝗻. 𝙏𝙧𝙖𝙞𝙣𝙞𝙣𝙜 𝙞𝙨 𝙖 𝙨𝙝𝙖𝙧𝙚𝙙 𝙧𝙚𝙨𝙥𝙤𝙣𝙨𝙞𝙗𝙞𝙡𝙞𝙩𝙮. The employer must provide structured, practical, repeated drills. The employee must participate seriously and be mentally ready to act. The hard facts: • Do not assume this will never happen to you. • Do not assume you will “manage it somehow.” • Do not assume instinct will replace training. 𝗙𝗶𝗿𝗲 𝗱𝗼𝗲𝘀 𝗻𝗼𝘁 𝗿𝗲𝘀𝗽𝗲𝗰𝘁 𝗰𝗼𝗻𝗳𝗶𝗱𝗲𝗻𝗰𝗲. 𝗜𝘁 𝗿𝗲𝘀𝗽𝗼𝗻𝗱𝘀 𝘁𝗼 𝗽𝗿𝗲𝗽𝗮𝗿𝗮𝘁𝗶𝗼𝗻. This incident ended without tragedy. That is good. But not every incident will be forgiving. Not every delay will be harmless. Not every fire will stay small. 𝗧𝗿𝗮𝗶𝗻. Emergencies are unavoidable—fires, floods, shootings, cyberattacks. The only thing worse than an emergency is being unprepared for it. Just ask yesterday's "Worst Employer" nominee. A well-crafted Emergency Action Plan (EAP) keeps everyone safe and your business running. Here's 10 things to consider in creating one: 1./ Assess Your Risks Identify the emergencies most likely to hit you—whether natural disasters, workplace violence, or data breaches. Prioritize based on impact and likelihood. 2./ Get Employee Input Your employees are on the front lines and often spot risks management misses. Including their insights builds a better plan and fosters buy-in. 3./ Assign Clear Responsibilities Who calls 911? Who initiates evacuations? Everyone should know their role before an emergency strikes to avoid confusion in the heat of the moment. 4./ Map Out Evacuation Plans Chart exits, evacuation routes, and assembly points. Make sure everyone can evacuate safely, including employees with disabilities. 5./ Establish Communication Channels Use multiple methods—emails, texts, and phone trees. Keep clients, vendors, and other stakeholders informed, too. 6./ Stock Emergency Supplies First-aid kits, fire extinguishers, and flashlights are must-haves. Regularly check supplies so nothing fails in a real emergency. 7./ Plan for Business Continuity Know which processes must keep running and how to do it—whether remote work, cloud backups, or backup vendors. 8./ Stay Compliant Verify if OSHA or other laws require specific elements in your plan. Non-compliance can mean fines. 9./ Train, Drill, and Support Your Team Hold regular drills, offer training refreshers, and provide mental health support after stressful events. 10./ Debrief, Report, and Improve After every emergency or drill, debrief with your team. File necessary incident reports for OSHA or insurance. Assign someone to review and update the plan regularly. Emergencies aren't predictable, but your preparation should be. A well-thought-out EAP protects your people and helps your business bounce back as quickly and easily as possible.

NIOSH has released two new documents addressing the health and safety of workers involved in emergency response and recovery efforts. The first, the NIOSH Emergency Responder Health Monitoring and Surveillance Primer (PDF), provides step-by-step guidance in implementing the Emergency Responder Health Monitoring and Surveillance (ERHMS) framework. This framework helps organizations, such as state, local, and tribal governments and community partners, ensure that response and recovery workers deployed to all types of emergencies have the necessary medical clearances, credentials, training, and equipment to do their work safely. The ERHMS Primer outlines the actions organizations should take before deployment, such as assessing workers’ fitness, providing training, and ensuring confidentiality; during deployment, such as documenting hazards, exposures, illnesses, and injuries; and after deployment, such as ensuring post-deployment assistance and continued medical monitoring. The appendix provides sample forms that may be used as they are or modified to fit an organization’s needs during each deployment stage. The second document, the third edition of Hurricane and Flood Key Messages for Employers, Workers, and Volunteers (PDF), is intended to help employers and workers, including volunteers, anticipate hazards before, during, and after disasters such as hurricanes and floods. This document also refers to the ERHMS framework and directs readers to the ERHMS Primer and EHRMS Technical Assistance Document (PDF). First, the Key Messages document summarizes employers and workers’ responsibilities at all stages of deployment, with links to relevant resources. These responsibilities include medical screening, personal hygiene, and personal protective equipment use.
