    AI is not failing because of bad ideas; it’s "failing" at enterprise scale because of two big gaps: 👉 Workforce Preparation 👉 Data Security for AI While I speak globally on both topics in depth, today I want to educate us on what it takes to secure data for AI—because 70–82% of AI projects pause or get cancelled at POC/MVP stage (source: #Gartner, #MIT). Why? One of the biggest reasons is a lack of readiness at the data layer. So let’s make it simple - there are 7 phases to securing data for AI—and each phase has direct business risk if ignored. 🔹 Phase 1: Data Sourcing Security - Validating the origin, ownership, and licensing rights of all ingested data. Why It Matters: You can’t build scalable AI with data you don’t own or can’t trace. 🔹 Phase 2: Data Infrastructure Security - Ensuring data warehouses, lakes, and pipelines that support your AI models are hardened and access-controlled. Why It Matters: Unsecured data environments are easy targets for bad actors making you exposed to data breaches, IP theft, and model poisoning. 🔹 Phase 3: Data In-Transit Security - Protecting data as it moves across internal or external systems, especially between cloud, APIs, and vendors. Why It Matters: Intercepted training data = compromised models. Think of it as shipping cash across town in an armored truck—or on a bicycle—your choice. 🔹 Phase 4: API Security for Foundational Models - Safeguarding the APIs you use to connect with LLMs and third-party GenAI platforms (OpenAI, Anthropic, etc.). Why It Matters: Unmonitored API calls can leak sensitive data into public models or expose internal IP. This isn’t just tech debt. It’s reputational and regulatory risk. 🔹 Phase 5: Foundational Model Protection - Defending your proprietary models and fine-tunes from external inference, theft, or malicious querying. Why It Matters: Prompt injection attacks are real. And your enterprise-trained model? It’s a business asset. You lock your office at night—do the same with your models. 🔹 Phase 6: Incident Response for AI Data Breaches - Having predefined protocols for breaches, hallucinations, or AI-generated harm—who’s notified, who investigates, how damage is mitigated. Why It Matters: AI-related incidents are happening. Legal needs response plans. Cyber needs escalation tiers. 🔹 Phase 7: CI/CD for Models (with Security Hooks) - Continuous integration and delivery pipelines for models, embedded with testing, governance, and version-control protocols.     15 weeks left before the first rules of the AI Act come into effect. Struggling with where to start on AI implementation and compliance? Start with a multidisciplinary team; conduct an AI inventory; carry out AI Impact Assessments; draft AI policies; amend contracts, policies, and data protection documents to reflect AI’s role in your organisation. Ensure your team is trained in AI literacy, as required under the AI Act. To navigate AI implementation and compliance under the EU AI Act, companies must begin by understanding its scope and risk-based approach. The Act categorises AI systems into prohibited, high-risk, or general-purpose. Prohibited AI systems (the first rules coming in) include those exploiting vulnerabilities or engaging in certain AI emotional recognition. High-risk systems, such as those used in management of critical infrastructure, require strict oversight, including documentation, risk assessments, and ongoing monitoring. General-purpose AI systems, widely used across industries, may also face regulatory scrutiny due to their broad impact. The first step for companies is conducting a comprehensive AI inventory. This involves cataloguing all AI systems in use or under development to determine their classification under the AI Act. Through this inventory, companies can assess their compliance obligations and identify any systems that may need modification or discontinuation to meet the Act’s standards. Data protection is a cornerstone of AI compliance. The AI Act mandates that data used in AI systems be high quality, representative, and free from bias. This is especially crucial for high-risk systems, which must undergo continuous risk assessments to protect fundamental rights. GDPR compliance is also essential for any AI system that processes personal data, and companies must ensure their data governance strategies focus on transparency, accountability, and safeguarding individual rights. Contracts are a critical component of AI implementation. Organisations must revisit and amend contracts to address how AI impacts their legal and operational frameworks. These amendments should explicitly cover liability for AI-generated decisions, intellectual property ownership of AI-generated outputs, and data protection compliance. Contracts must minimise legal exposure. Additionally, intellectual property issues around AI, such as ownership of outputs or the use of third-party data, should be clearly defined in these agreements. Following the AI inventory, companies must conduct an AI impact assessment. This assessment includes both a Data Protection Impact Assessment (DPIA) and a Fundamental Rights Impact Assessment (FRIA). The extraterritorial scope of the AI Act means that even non-EU companies must comply if their AI systems impact the EU market. It also outlines the evolving risk landscape that organizations must navigate as they scale their use of AI.     Your board does not have an AI risk framework. If it did, someone would have asked about it in the last six months. Three in four boards have approved major AI investments. Fewer than half have set governance expectations for them. (Grant Thornton, 2026.) Usage is not the problem. Most companies already run AI somewhere — often in more places than the board has been told. Sit in a board meeting and you will hear adoption numbers and a slide about productivity gains. Everyone nods, and the agenda moves on. Adoption is not governance. Governance is the part where the board can answer who is accountable when an AI-driven decision goes wrong. Ask that in most boardrooms and you get a pause — then a look towards whoever manages IT. A real framework has five parts, and most companies have none of them: ✅ A model inventory: every AI system you actually run, including the tool your CMO bought on a credit card ✅ A human-in-the-loop policy: which decisions need a person to sign off, and which do not ✅ An incident playbook: what happens when the model misbehaves at 11pm on a Sunday ✅ Vendor accountability: when a third-party AI breaks, the accountability is still yours. PDPA does not let you contract it away ✅ A decision audit trail: could you reconstruct a customer-impacting AI decision if a regulator asked tomorrow · Frontier models change weekly. Your board meets quarterly. That maths only works if governance is structural: checked at every meeting, owned by a named director, stress-tested at least once a year. If you want a starting point that already exists, look at IMDA's Model AI Governance Framework. It now covers AI agents specifically. The boards that sort this out in 2026 are the ones not writing apology letters in 2027. Take those five into your next board meeting and count how many you can answer without leaving the room.     Let me explain... ▶️ Attackers Are Weaponizing Trust Itself Cyber criminals are increasingly focusing on getting better at hijacking trust signals that fool users into taking harmful actions, developers into downloading harmful packages, etc. Worse off, we've spent years, training users to rely on and look out for the very trust signals that attackers are getting better at convincingly mimicking. Consequently, traditional security tools are being bypassed ever more often. Trust is broken! ▶️ Trust Transcends Perimeters In modern architectures, trust lives in identities, tokens, APIs, supply chains, and even human relationships. When we grant an application, partner, or employee a high level of trust, we're effectively enlarging our “attack surface” to WHEREVER that trust extends. A compromised cloud credential or an abused API token can bypass traditional defenses undetected, because the system assumes “trusted” traffic is not harmful. ▶️Supply-Chain Dependencies Each third-party library, managed service, or vendor relationship is a trust link; a vulnerability or breach in any link immediately widens the attacker’s reach into your environment. ▶️The Zero Trust Paradox The rise of “zero trust” architectures means every request must be authenticated, every session evaluated, every transaction authorized. Ironically, the constant negotiation of trust doubles as an attack surface. Here's why; if your policy engine or identity provider is misconfigured, overloaded, or compromised, attackers can gain unfettered access. So here's my prognosis: - Expect adversaries to increasingly target IAM systems, API gateways, and CI/CD pipelines, exploiting the very mechanisms organizations rely on to grant access and permissions. - Personalized deep fake attacks will surpass mass phishing by 2027. - Discerning leaders will deploy tools that operationalize context at scale. CONTEXT IS NOW KING!!! Organizations will shift to context-aware trust assessments; monitoring behavioral anomalies, device posture, and risk signals at every transaction to detect misuse of “trusted” assets. - As orchestration tools become universal, attackers will shift to poisoning CI/CD pipelines. A malicious change to a shared workflow or action could inject backdoors into every deployment, turning your “automation trust” into a systemic vulnerability. In fact, Gartner predicts a 50% rise in breaches traceable to vendor software flaws or misconfigurations. - By 2026, both defenders and attackers will leverage AI for behavior modeling. Attackers will focus on “data poisoning”, through faux-legitimate actions making anomaly detection. Building Trust Is The Only Future That Matters!

    I've reviewed Anthropic's Risk Report for Claude Opus 4.6 because many of our enterprise customers are actively deploying AI agents into production environments. When those systems fail, the consequences are operational, financial and reputational. Most of the reaction centers on the headline that catastrophic risk is very low but not negligible. What matters more for customers and future customers is how risk actually manifests inside live enterprise systems and what that means for uptime, data integrity and compliance. It does not look like a breach. It looks like business as usual. An agent subtly influencing procurement decisions. A finance workflow that starts omitting inconvenient data. Permissions that expand over time without clear oversight. Anthropic describes a scenario called Persistent Rogue Internal Deployment, where an AI system with privileged access creates a less monitored instance of itself and continues operating inside production systems. In a real enterprise environment, that translates into downtime, data exposure or regulatory impact. The organizations at greatest risk are not the ones moving cautiously. They are the ones who pushed agents into production without adding an operational governance layer. We have seen this pattern before in cloud adoption. Technology advances quickly, and controls often lag behind. That gap is where exposure grows. So what should enterprise IT and security teams do now? 1. Constrain actions, not just access. Define what an agent can set in motion and enforce least privilege at the identity level, just as you have done for human users for decades. 2. Log actions, not just outcomes. Maintain an auditable trail of what the agent did, where and what triggered it, the same standard applies to human operators in regulated environments. 3. Automate your tripwires. Do not rely on people to catch machine speed behavior. Build policy enforcement and anomaly response into the loop. 4. Audit your agent footprint. Inventory every agent, its owner, permissions and kill path. Governance starts with visibility and most enterprises are still building it. The window to build these guardrails is now, before the agent workforce scales. At Rackspace, 25 years of running mission-critical systems have taught us that trust without controls creates exposure.     Every AI failure you've read about traces back to one of these risks. Not a bug. Not bad luck. A known, named, predictable category of risk that every AI team should already be tracking. Here's the AI Risk Periodic Table, mapped across 10 categories every founder, product leader, and enterprise team needs to understand. 𝟭. 𝗠𝗼𝗱𝗲𝗹 𝗥𝗶𝘀𝗸𝘀 Hallucination, bias, drift, overfitting, underfitting, error propagation. The model itself fails before anyone touches it. 𝟮. 𝗗𝗮𝘁𝗮 𝗥𝗶𝘀𝗸𝘀 Mislabeling, source risk, synthetic data risk, duplicate data, data leakage, consent risk, quality loss. Bad data breaks good models. 𝟯. 𝗦𝗲𝗰𝘂𝗿𝗶𝘁𝘆 𝗥𝗶𝘀𝗸𝘀 Jailbreaks, prompt injection, adversarial attacks, API abuse, token theft, supply chain risk. Every AI system is a new attack surface. 𝟰. 𝗢𝗽𝗲𝗿𝗮𝘁𝗶𝗼𝗻𝗮𝗹 𝗥𝗶𝘀𝗸𝘀 Scaling, cost overrun, latency, deployment, documentation, integration, rollback gaps. Where production AI quietly bleeds money. 𝟲. 𝗕𝘂𝘀𝗶𝗻𝗲𝘀𝘀 𝗮𝗻𝗱 𝗥𝗲𝗽𝘂𝘁𝗮𝘁𝗶𝗼𝗻 𝗥𝗶𝘀𝗸𝘀 Reliability, reputation, customer trust loss, revenue impact, ROI failure, strategy misalignment. The risks the CFO cares about most. 𝟳. 𝗛𝘂𝗺𝗮𝗻 𝗮𝗻𝗱 𝗘𝘁𝗵𝗶𝗰𝗮𝗹 𝗥𝗶𝘀𝗸𝘀 Fairness, trust gap, ethical risk, automation bias, job displacement fear. The risks that decide whether anyone actually uses your AI. 𝟴. 𝗙𝗮𝗶𝗹-𝗦𝗮𝗳𝗲 𝗥𝗶𝘀𝗸𝘀 Kill switch gap, feedback gap, evaluation failure, red teaming gap. The layer that decides whether AI fails gracefully or catastrophically. 𝗧𝗵𝗲 𝗯𝗶𝗴 𝗶𝗱𝗲𝗮: Most AI teams worry about hallucinations. The best teams worry about all 70+ of these, with a system to monitor each one. AI isn't risky because it's new. Understand the specific legal requirements and privacy regulations that apply, such as GDPR or the California Consumer Privacy Act. 2. For PII, such as names, addresses, or social security numbers, consider redacting this information before making API calls, especially if the data could be linked to sensitive applications, like healthcare or financial services. 3. Avoid Sharing Highly Sensitive Information Never pass sensitive personal information, such as credit card numbers, passwords, or bank account details, through AI endpoints. Instead, use secure, dedicated channels for handling and processing such data to avoid unintended exposure or misuse. 4. Implement Data Anonymization When dealing with confidential information, like health conditions or legal matters, ensure that the data cannot be traced back to an individual. Anonymize the data before using it with AI services to maintain user privacy and comply with legal standards. 5. Regularly Review and Update Privacy Practices Data privacy is a dynamic field with evolving laws and best practices. To ensure continued compliance and protection of user data, regularly review your data handling processes, stay updated on relevant regulations, and adjust your practices as needed. Remember, safeguarding sensitive information is not just about compliance — it's about earning and keeping the trust of your users.
